Working across community and hospital systems, Solid Start promotes patient-centered whole-person care, health equity, innovations in service delivery, integration of health and social services, multi-agency collaboration, and increased access to services and resources through integrated care coordination.
Social Determinants of Health
Social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, and quality-of-life outcomes and risks.
Systems Change
In order to solve complex problems we need to see and leverage the entire system. However, the language of systems change can sometimes be abstract, making it hard to get started. Fortunately, collective impact provides us with a familiar and accessible way to approach systems change.

Large-scale social change requires broad cross-sector coordination, yet the social sector remains focused on the isolated intervention of individual organizations.
Community Health Workers
Community Health Works (CHWs) are trained public health workers who serve as a bridge between communities, health care systems, and state health departments. CHWs are chosen by community members or organizations to provide basic health and medical care, and are capable of providing preventive, promotional and rehabilitation care to that community.
